"The UnitedHealthcare Nursing Home Plan uses nurse practitioners who coordinate with nursing home staff. The nurse practitioners offer personalized, care coordination services to nursing home residents. They also support the efforts of your nursing home staff by providing enhanced communication with the entire nursing home team."
For more information about this program and how it may benefit your loved one, please ask our social workers for a brochure or ask for our UnitedHealthcare Nurse Practitioner, Margo Packheiser.

For Your Information
Did you know Clapp's is a Restraint Free Facility? We provide your loved one with a safe, homelike environment free from devices designed to restrain movement.
Clapp's also has a No Lift Policy. Resident's are assisted in transferring but are not lifted by staff. Resident's who are unable to transfer with assistance are assisted via mechanical lifts which are safer for the resident and prevent injuries to the staff.
If you have questions about other policies, please ask our social workers Alison Kennedy or Elizabeth Santore.

Pets in the Facility
Clapp's Nursing Center is a pet friendly facility. We provide pet therapy twice a month in addition to having two small dogs that are a comfort to many of our residents during the week. We encourage you to bring your pet provided the pet is housetrained, on a leash at all times and you bring a copy of the pets rabies vaccination record in to the Activity Director prior to bringing in the pet. Also, no pet is allowed in the Dining Room at any time - no exceptions. This is a health code regulation.

Service-Learning Program
Clapp's Nursing Center is a proud participant of Guilford County's High School Student's Service-Learning Program. Through this program students have the opportunity to receive a Service-Learning Diploma and Exemplary Award. Student's participate in five components: investigation, preparation & planning, action, reflection and demonstration. This program extends beyond volunteer service of a few hours or that which is needed to complete a senior project. Through this program students begin volunteering as early as ninth grade and have the opportunity to volunteer at several Service-Learning Program Providers to gain a variety of experiences while developing their skills and character. The relationship the student develops with the service learning provider is one of a mentorship. The volunteer hours can be used when applying for college.
The Service-Learning Program is a relatively new program which is being promoted in high schools. Students are required to complete a Service-Learning Application. To request a handbook or for more information contact: Yvonne Eason, Coordinator of Character Development and Service Learning at 370-2330.
Participants are required to complete Clapp's volunteer orientation, pass a TB test, volunteer when activity staff are scheduled, be at least 13 years old and volunteer with the activity department only.

Insurance Information
Many insurance companies have open enrollment annually. If your loved ones insurance changes, including prescription drug plans, please inform the social workers. There are some plans Clapp's Nursing Center does not participate in and therefore cannot accept. Our facility will need a copy of the new insurance card. Our receptionist is happy to make a copy for you. Thank you!
